In order for us to change your address, kindly send us your new lease agreement/ proof of ownership and the OHS compliance certificate for the new premises not older than 12 months.
In the body of the e-mail, you must also provide your new physical address as well as your postal address when sending these documents through.
Furthermore, a site verification visit may be conducted at the discretion of the ICB.
No address will be changed if the ICB has not received the stated documents.
